The Challenge

How might we electronically digitize registration for a meals on wheels service?

You are a designer with the municipal government. You need to create a form that will allow town residents to apply for a meals on wheels service.

Be sure to consider UX patterns and principles: hierarchy of the content, digital affordances, pliancy, how type and color can support, and other Interaction Design principles within simplicity, consistency, error prevention, detection and recovery.
